Doctor certifies the type of service needed. One or more of these: Intermittent skilled nursing care, Physical and or Occupational therapy.
Collect adequate documentation from doctor or other healhcare professional to determine eligibility.
Doctor must certify that you are homebound, meaning you have trouble leaving your home due to illness or injury.
Work in conjunction with Doctor to develop a care plan that best fits the patient’s treatment.
